5. This website is not a patient portal

Information submitted through this website is used to start a conversation, not to deliver care. Clinical records created in the course of actual treatment are maintained separately by the clinic and handled in accordance with applicable law. Do not submit urgent medical concerns through this website — for emergencies, call 911.
